Platinum Dental Milling offers a customer loyalty rewards program. This simple program allows you to earn 1% cash back in the form of a prepaid VISA Gift Card. The value of your VISA Gift Card is based on your monthly purchases with Platinum Dental Milling. You are automatically participating in this program just by outsourcing your milled zirconia, PMMA, and wax! Minimum monthly purchase to earn a reward is $1000.00. Rewards do not accumulate from month to month. At the beginning of each month, you will receive a gift card based on the previous month's purchases as long as your statement demonstrates a minimum purchase of $1000.00. Your card will be ordered and sent to you automatically. You don't have to do anything extra! Just outsource your milled zirconia, PMMA, and wax to Platinum Dental Milling! Example: If you spend $2357.00 the previous month with Platinum Dental Milling, you will receive a VISA gift card in the amount of $23.57. If you spend $873.00 the previous month, you will not qualify for a reward for that month's activity. The card expires 12-months from the purchase date. In the event the card expires, the recipient may contact the issuing company for a replacement card minus a $5.00 processing & shipping fee. Terms subject to change without notice.
